Defining Attention Deficit/Hyperactivity Disorder
The DSM-IV categorically sorts the symptoms of AD/HD into three subtypes:

*drawn from the APA (1994) Diagnostic and Statistical Manual of Mental Disorders
1. Combined Type: multiple symptoms of inattention, impulsivity and hyperactivity
2. Predominantly Inattentive Type: multiple symptoms of inattention with few, if any, of hyperactivity-impulsivity.
3. Predominantly Hyperactive-Impulsive Type: multiple symptoms of hyperactivity-impulsivity with few, if any, of inattention.

Other essential diagnostic features of AD/HD

Symptoms of inattention, hyperactivity, or impulsivity must persist for at least six months and be maladaptive and inconsistent with developmental levels.
Some of the symptoms causing impairment must be present before age 7 years.
Some impairment from the symptoms is present in two or more settings (for example: at school or work, at home, at social functions).
Evidence of clinically significant impairment is present in social, academic or occupational functioning.
Symptoms do not occur exclusively during the course of Pervasive Developmental Disorder, Schizophrenia, or other Psychotic Disorder and are not better accounted for my another mental disorder.

Inattention

Hyperactivity

Impulsivity

Children with AD/HD may have difficulty with one or all parts of the attention process. Some may have difficulty concentrating on tasks, others may have trouble knowing where to start a task, others may get lost in the direction along the way.
Symptoms of inattention include:
a) often fails to give close attention to details or makes careless mistakes in schoolwork.
b) often has difficulty sustaining attention in tasks or play activities.
c) often does not seem to listen when spoken to directly.
d) often does not follow through on instructions and fails to finish schoolwork, chores, or duties.
e) often has difficulty organizing tasks and activities.
f) often avoids, dislikes, or is reluctant to engage in tasks that require sustained mental effort.
g) often loses thing necessary for tasks or activities.
h) is often easily distracted by extraneous stimuli.
i) is often forgetful in daily activities.
Excessive activity is the most visible sign of AD/HD. The hyperactive toddler/ preschooler is generally described as "always on the go" or "motor driven". By adolescence/adulthood, overactivity may appear as restless, fidgety behavior.

Symptoms of hyperactivity include:
a) often fidgets with hands or feet or squirms in seat.
b) often leaves seat in classroom or in other situations in which remaining seated is expected.
c) often runs about or climbs excessively in situations in which it is inappropriate.
d) often has difficulty playing or engaging in leisure activities quietly.
e) is often "on the go" or often acts as if "driven by a motor".
f) often talks excessively.

From time to time, all children will be inattentive, impulsive, and overly active. In the case of AD/HD, these behaviors are the rule not the exception!
Children with AD/HD tend to act before thinking because they have difficulty waiting or delaying gratification. The impulsivity leads these children to speak out of turn, interrupt others, and engage in what looks like risk-taking behavior.

Symptoms of impulsivity include:
a) often blurts out answers before questions have been completed.
b) often has difficulty awaiting turn.
c) often interrupts or intrudes on others.

It is important to note that in the DSM-IV, hyperactivity and impulsivity are no longer considered as separate features.
In addition to problems with inattention or hyperactivity-impulsivity, the disorder is often seen with associated features. Parents or teachers may see low frustration tolerance, temper outbursts, bossiness, difficulty in following rules, disorganization, poor self- esteem, etc.

Indicators of Physical Child Abuse
The statistics on physical child abuse are alarming! Of the estimated hundreds of thousands of children battered each year by a parent or close relative, thousands die. For those who survive, the emotional trauma remains long after the external bruises have healed.

Children who have been physically abused may display:

- A poor self-image
- Inability to depend on, trust or love others
- Aggressive and disruptive (sometimes illegal) behavior
- Passive and withdrawn behavior; fear of entering into new relationships or activities
- School failure
- Serious drug and alcohol abuse
Indicators of Child Sexual Abuse
The behavior of sexually abused children may include:
- Unusual interest in or avoidance of all things of a sexual nature
- Sleep problems, nightmares
- Depression or withdrawal from friends or family
- Seductiveness
- Statements that their bodies are dirty or damaged
- Fear there is something wrong with them in the genital area
- Refusal to go to school, delinquency
- Secretiveness
- Aspects of sexual molestation in drawings, games, fantasies
- Unusual aggressiveness
- Suicidal behavior
- Other severe behavior changes
